ADA Grievance Procedures

ADA Grievance Procedures

Concerns about a program, service, or activity in the City of Richfield that is not accessible to persons with disabilities should be directed to Brittany Bartlett, city ADA Coordinator, via email. The City of Richfield is committed to making all city programs, services, and activities accessible to everyone. Most concerns can be remedied internally between residents and city department staff. If you would like to file a formal complaint, please download a Grievance Form, and return it to the ADA Coordinator via email or mail. Alternatively, you can also fill out an electronic Grievance Form here. If sending in a form other than the Grievance Form, please include all of the following information:

- Name, address and telephone number of the person filing the grievance
- Name, address and telephone number of the person alleging the ADA violation, if other than the person filing the grievance
- A description of the alleged violation and the remedy sought
- Information on whether a complaint has been filed with the Department of Justice or other federal or state civil rights agency or court.
- If a complaint has been filed, the name of the agency or court where the complaint was filed, the date the complaint was filed, and the name, address and telephone number of a contact person with the agency with which the complaint was filed.

The written complaint should be submitted as soon as possible, but no later than 60 calendar days after the alleged violation.
